〈역자서문〉

초음파검사는 오늘날 임상 현장에서 가장 널리 사용되면서도, 동시에 가장 배우기어려운 검사 중 하나입니다. 교과서에는 분명히 그림이 있고 설명도 충분한데, 막상프로브를 손에 쥐고 환자 앞에 서면 "지금 내가 보고 있는 이 영상이 과연 맞는 것일까?"라는 질문 앞에서 누구나 한 번쯤 멈춰 서게 됩니다.

저 역시 임상에서 초음파검사를 처음 접했을 때, 해부학은 알고 있는데 영상이 이해되지 않고, 영상은 보이는데 어떻게 스캔해야 같은 화면이 나오는지 알 수 없었던경험을 반복했습니다. 그때 절실히 깨달았던 것이 있습니다. "초음파검사는 아는 만큼 보인다는 것"입니다. 그 간극을 메워 줄 책을 찾는 일은 생각보다 쉽지 않았습니다.

이 책은 바로 그 간극을 정확히 메우기 위한 초음파 참고서입니다. 단순히 "어디를보면 무엇이 보인다"를 설명하는 책이 아닙니다. 초음파 빔이 어느 방향에서 들어가,어떤 해부 구조를 통과하며, 그 결과 화면에 어떤 영상이 만들어지는지를 에코 영상,해부도, 도식, 실제 수검자의 사진을 하나의 세트로 제시함으로써 '보는 순간 이해되는 초음파'를 가능하게 합니다.특히 이 책의 가장 큰 강점은 여러 장기와 검사 영역을 한 권으로, 동일한 사고 체계로 배울 수 있다는 점입니다. 건강검진기관에서 선별검사부터 응급실과 외래 진료실에서 즉각적인 평가, 수술 전 검사에 이르기까지, 실제 초음파검사는 특정 장기만 검사하는 경우보다 여러 영역을 동시에 익혀야 하는 상황이 훨씬 많습니다. 이 책은 그러한 실제 임상 환경을 정확히 반영하여 초심자가 어디서부터 무엇을 어떻게 시작해야 하는지를 명확하게 제시합니다.

"이 병변을 만났을 때 무엇을 봐야 하는가", "어떤 소견이 핵심인가", "판독 보고서에는 어떻게 표현해야 하는가"가 직관적으로 이해되도록 구성되어 있어 단순한 참고서를 넘어 현장에서 바로 활용할 수 있는 실전 지침서의 역할을 합니다.초음파검사를 처음 시작하는 검사자, 그리고 이미 현장에서 초음파검사를 하고 있지만 기본을 다시 정리하고 싶은 모든 분들께 이 책이 든든한 출발점이 되기를 바랍니다.

〈저자 서문〉

2014년에 발간된 초판본은 초음파검사를 시작하려고 하는 검사자뿐만 아니라, 수련의와 학생 등, 매우 많은 분들에게 인기를 얻었습니다. 그리고, “이해하기 쉬워서 탐촉자를 직접 사용해 보고 싶은 책”, “입체감각을 상상하기 쉽고, 초심자에게 안성맞춤”, “깔끔하고 정돈되어 있어 보기 쉽다” 등 다수의 리뷰 코멘트를 받았습니다. 예상을 뛰어넘은 반응이었고, 집필자 모두 대단히 기쁘게 생각하고 있습니다.

그리고, 이번에 제2판을 발간하게 되었습니다. 8년 동안에 3차원 초음파, 스트레인 초음파, 엘라스토그래피(Elastography, 탄성영상검사) 등의 새로운 기술이 발전하고 일상 검사에도 도입하게 되었습니다. 그렇지만, 이 책에서 이 항목들을 추가하지 않았습니다. 왜냐하면, 기본적인 초음파검사 방법만 마스터하고 있으면, 새로운 기술에도 바로 대응하는 것이 가능하기 때문입니다.

이 책의 특징은 무엇보다도 초음파 영상, 도식, 해부도, 피검자 사진을 4종 세트로 하고 있습니다. 초음파 빔이 어디에서 어떻게 장기를 횡단하고 있으니까 초음파에서 어떤 영상으로 나타나는지를 직관적으로 이해할 수 있도록 되어 있고, 참으로 초심자에게 안성맞춤인 한 권의 책으로 구성되어 있습니다.
이 책은 “보면 알 수 있다. 판독하면 더 잘 알 수 있다”라는 컨셉은 유지하고, ① 기본적인 촬영법의 부족한 부분을 추가, ② 초음파 영상의 일부를 보다 깨끗한 것으로 변경, ③ 비정상 소견을 추가, 변경하고, 보다 완성도를 높였습니다. 초음파검사는 자신의 손으로 탐촉자를 잡고 영상을 획득하는 유일무이의 영상진단입니다. 초음파검사를 시작한 여러분들에게 초음파를 촬영할 수 있으면 좋겠다, 초음파검사의 매력을 느끼고 싶다, 더 깨끗한 영상을 획득하고 싶다 라는 염원은 제2판에서도 퇴색될 수 없습니다.

어엿한 초음파검사자가 되려면, 많은 증례를 경험하고, 많은 기술과 지식을 몸에 익히지 않으면 안 됩니다. 그것을 위한 토대를 만드는 데 이 책이 조금이라도 도움이 된다면 기쁘겠습니다.
