Ward Clerk: Good morning, Ward 8. Ward clerk, Jane speaking. How can I help you?
Ben: Good morning, Jane. This is Ben speaking. I’m a surgical house officer. Can I talk to the charge nurse, Peter, please? He paged me just before.
Jane: You’re right. He’s just sitting here and staring at me now. One moment.
Peter Thanks, Jane. Hi, Ben. How’re you?
Ben: Steady busy but not bad. How about you?
Peter: I’m pretty alright, thanks. Do you know Mrs. Catherine Reid? She’s been admitted this morning from ED for her right arm infection because of cat bites.
Ben: Yes, I know her. She’s in Room 5, isn’t she? Can you just tell me her NHI number for sure?
Peter: Of course. It’s Alpha, Charlie, Tango 456. Her name is Catherine Reid and she’s 75 years old.
Ben: Thank you. How can I help her?
Peter: She’s here with her daughter who was concerned about her mother’s urinary retention and had me scan her bladder. As a result, I found she has more than a litre of urine in her bladder, but she hasn’t felt anything. I mean she doesn’t feel any urgency or desire to void at all. She had a similar episode during her last admission for a chest infection which was last year. According to her, she usually passes urine four times a day or more and she hasn’t had any problems in her water works at home. However, she hasn’t been to the toilet since last night and it’s almost midday. I think she’ll need an indwelling catheter insertion as soon as possible. I suggested that she go to the toilet and try to pass urine while I’m discussing this with you. What do you think?
Ben: I agree with you. She’s definitely got urinary retention with a large volume. Can you please insert a Foley catheter now? I’m quite busy at the moment in Ward 5 but I think I can assess her within one hour. Is it okay for you?
Peter: That won’t be a problem for me. I’m going to chase her up about whether she passed urine and if not, I’ll insert the catheter. I’ll talk to Catherine and Susan about what we’ve discussed and tell them you’ll see Catherine within one hour. I’ll also send a urine sample if her urinalysis result is abnormal.
Ben: Thanks, Peter. Before you hang up the phone, can you find RN Amitha? I just received another page from her while we were talking.
Peter: Sure, she’s in the queue now. Thanks, Ben. Hang on a second. Amitha is right next to me. It’s your turn, Amitha.
Amitha: Hi, Ben. This is Amitha. Thank you for replying so quickly.
Ben: Hi, Amitha. What’s up?
Amitha: It’s not urgent. One of my patients will need a new IV cannula for IV antibiotics. He is Mr John Smith in Room 12-1. I texted you his NHI number and other details through your pager. Can you find the details?
Ben: Yes, Mr John Smith. His NHI number is MTU996, isn’t it?
Amitha: That’s him. The current IVC was tissued so I had to remove it. His next IV antibiotics is due at 2: 30pm. When you cannulate a new one, can you use a bigger cannula than 21 gauge because he’s on the list for a CT scan tomorrow. They usually request that we insert a bigger gauge of IV cannula for CT contrast administration. He’ll also need another IV fluid charted as he is still nil by mouth.
Ben: I can do those. Amitha, I can’t be there right now but within an hour. Is that okay for him?
Amitha: That’s no problem. He’s taking a shower now and he’s enjoying being free from the attachments. I don’t want to interrupt his free moment.
Ben: That sounds good. Is that all from you?
Amitha: Yes, that’s all from me so far but if I page you again, please forgive me.
Ben: I will. See you later then. Bye.

병동 사무원: 좋은 아침입니다. 8병동 사무원 제인입니다. 무엇을 도와드릴까요?
벤: 안녕하세요 제인. 의사 벤입니다. 수간호사 피터 선생님과 통화할 수 있을까요? 좀 전에 호출하셨거든요.
제인: 맞네요. 지금 바로 제 옆에 앉아 계세요. 잠시만 기다리세요.
피터: 고마워요 제인. 안녕하세요 벤 선생님. 잘 지내셨나요?
벤: 좀 바쁘지만 나쁘지 않습니다. 선생님은 어떠세요?
피터: 전 괜찮게 잘 지냅니다. 고마워요. 캐서린 레이브 부인 아시나요? 고양이한테 물리셔서 오늘 아침 응급실 통해서 입원하셨는데.
벤: 네 알아요. 5호실에 계신 분, 맞죠? 정확히 해야 하니까 환자번호 좀 알려 주세요.
피터: 네, Alpha, Charlie, Tango 456. 성함은 캐서린 레이드, 75세 여성분입니다.
벤: 감사해요. 뭘 도와드릴까요?
피터: 그 환자분 따님이 어머니 요폐 증상이 있는 것 같아 걱정하셨어요. 그래서 스캔해보니 방광에 소변이 1리터 이상 있었어요. 근데 정작 환자분은 별 느낌이 없으신 거죠. 요의나 긴박뇨 같은 느낌이 하나도 없으시대요. 작년에 폐렴으로 입원하셨을 때도 비슷한 경험을 하신 모양입니다. 환자분에 따르면 하루에 4번 이상 소변을 보시고 집에 계실 땐 별다른 문제가 없으셨다고 하고요. 근데 지금이 벌써 정오인데 어젯밤부터 화장실을 한 번도 안 가셨대요. 그래서 가능하면 빨리 소변줄을 삽입해야 할 것 같아요. 선생님과 상의해 보는 동안 일단은 화장실 가셔서 소변을 보도록 해보시라고 말씀은 드렸고요.
벤: 저도 같은 생각입니다. 확실히 요폐가 있으시네요. 지금 소변줄 삽입해 주실 수 있나요? 제가 지금 5병동에서 좀 바빠서 한 시간 안에 그 환자분 볼게요. 괜찮으시겠어요?
피터: 문제없습니다. 화장실에서 소변을 보셨는지 확인해보고 아직도 못 보셨으면 삽입할게요. 환자분과 따님께는 지금 상의한 것과 한 시간 안에 의사 선생님께서 보러 오신다고 설명할게요. 소변 검사 해보고 결과가 비정상이면 소변 샘플 보낼게요.
벤: 고마워요. 피터 선생님. 아 끊기 전에 아미싸 간호사님 좀 바꿔주세요. 통화 중일때 호출을 받았어요.
